HORIZON BLUE CROSS BLUE SHIELD (Traditional Networks - NO Medicare or Medicaid)
For all other insurance companies, I am an Out-Of-Network provider. That means that I do not participate with any insurance company, however, you will be provided with what is called a "super bill". A super bill is a receipt for payment which you can submit to your insurance company for reimbursement. Please check your benefits and eligibility with your insurance company.
Policies
Cancellation Policy
I have a 24-hour cancellation policy. This means, you may cancel or re-schedule your appointment with a 24-hours notice. Should you miss your appointment without canceling or re-scheduling, a $ 50 cancellation fee will be charged to your credit card on file.
Emails
All emails will be answered once per week or earlier if time permits.
Payments including co-pays
Payment is expected at the end of each session and can be made either via Venmo, Zelle, PayPal, or Credit Card. Credit Card payments will require a 3.75% service fee. Should you require a session yet are not able to pay the fee, kindly let me know prior to scheduling. I will work with you to find a solution for you to be able to attend your session.
Above rates also apply to other professional services that you may need, including telephone consultations of more than 10 minutes, report writing, preparation of records or treatment summaries, travel time, etc., unless we agreed upon otherwise. In special cases, home visits can be arranged upon request. An additional charge applies depending on the travel time to/from the location.
*Sliding scale is available upon request for individuals without insurance. Sliding Scale depends on clients income.
Phone Calls
All phone calls will be returned within 24 hours if client remarks them as "urgent". Otherwise, all calls will be answered when time permits. Sometimes the response is made via text message.
Text Messages
All text messages will be answered when time permits it. Text messages are to be used to get in touch with clinician yet not as a tool to vent or convey grievances, disappointment etc. The content of text messages will be discussed during session and not via text message. In case a client is texting the clinician any statements of self-harm or suicidal / homicidal content, the clinician reserves the right to inform authorities. In such an instance, the police will be notified to conduct a wellness check.
